Thank you for your interest in signing up your business to become a Business/Education Partner.

Please check the boxes and fill in fields below that best describe the type of partnership you would like. Once we have your information, we will forward it to a representative of the appropriate Business/Education Partnership Program in your area who has the authority to approve or reject your application.

1.	PARTNERSHIP TYPES
	Check as many boxes below as apply to you:
	TYPE A: One-Time, In the Classroom
	Check if applicable	Teacher selects business/community/parent to speak to class on a specific topic Approximately 1 hour of commitment Possibly several times a year In support of specific curriculum goals Under supervision and guidance of teacher This would result in the initiation of a Project Partnership between the business/community entity and the educational group
	TYPE B: One-Time, In the Workplace
	Check if applicable	Teacher selects business for workplace visit 1-2 hours of commitment Possibly several times a year In support of specific career training goals Under supervision and guidance of teacher Career-related agenda selected by employer, under approval of teacher This would result in the initiation of a Project Partnership between the business/community entity and the educational group
	TYPE C: Recurring, In the Classroom
	Check if applicable	Either teacher or business proposes on-going in-class program Teacher and business work together to refine the proposal so that it works for all Topic covered should supports specific curriculum goals Several recurring meetings throughout a semester / term Under supervision and guidance of teacher This would result in the initiation of a Standing Partnership between the business/community entity and the educational group
	TYPE D: Recurring, In the Workplace
	Check if applicable	Business proposes workplace program Teacher and business work together to refine the proposal so that it works for all Topic covered should supports specific career goals Several recurring meetings throughout a quarter or year Under supervision and guidance of teacher This would result in the initiation of a Standing Partnership between the business/community entity and the educational facility
